In order for your abstract to be reviewed, you and co-author(s) (if applicable) must complete the Biographical Data and Disclosure Form.
If you are submitting more than one abstract, you will only need to complete one Biographical Data and Disclosure Form, yet you MUST include all abstract topics on the Disclosure Form.
